Federal laws require that employers hire only individuals who are authorized to be lawfully employed in the United States. In compliance with such laws, all offers of employment are subject to verification of each applicant’s identity and employment authorization, and it will be necessary for you to submit such documents as are required by law to verify your identification and employment authorization upon employment.

Additionally, Bluco Corporation may procure or have prepared an investigative consumer and/or credit check. I hearby release Bluco Corporation from any and all liability for damages arising from furnishing the requested information.

I understand that Bluco Corporation follows an “employment at will” policy, in that I or Bluco Corporation may terminate my employment at any time or for any reason consistent with applicable state or federal law. This “employment at will” policy cannot be changed verbally or in writing, unless the change is specifically authorized in writing by the President of Bluco Corporation. I understand that this application is not a contract of employment, nor an offer of employment.

I understand and agree that I may be required to take a full physical examination which may include tests for substance abuse as a condition of hiring or continued employment. I understand and agree that any offer of employment I may receive from Bluco Corporation is contingent upon my successful completion of Bluco Corporations total pre-employment screening process, including Bluco Corporation receiving references that it considers satisfactory, and my satisfactory completion of any post-offer, pre- employment medical examination that Bluco Corporation my require.

I authorize an investigation of all statements contained herein and the references listed above to give you any and all information concerning my previous employment and any pertinent information they may have, personal or otherwise, and release all parties from all liability for any damage that may result from furnishing same to you.

I understand this application will be active for a period of 6 months; after that time, if I wish to be considered for employment, I must submit a new application. I understand that Bluco Corporation will thoroughly investigate my work and personal history and verify all data given on this application, on related papers and in interviews. I authorize all individuals, schools, and firms named therein, except my current employer if so noted, to provide any information requested about me, and I release them from all liability for damages in providing such information.

I certify that all of the foregoing statements are true and complete and understand that any falsification or willful omission shall be sufficient cause for denial of employment, or immediate termination for cause.
